Infrared Sauna
If you are scheduled for an Infrared Sauna session, please review and complete the form in this section. This form only has to be completed once, prior to your initial session. Feel free to submit completed forms electronically anytime prior to your appointment, or you may print and bring the completed forms with you to your appointment.
Membership Agreement
The Membership Agreement must be completed by those who have selected/purchased any membership or package. Memberships exceeding a 1 month term require the entire form be completed, including the EFT/Billing Section of the form. Those purchasing a 1 month membership or any package are required to complete the Plan Holder's Information Section, initial in agreement with the terms & provide signature .
ONDAMED
If you are scheduled for an ONDAMED Session, please complete the appropriate form/s in this section prior to your appointment. These forms only have to be completed once & are only required prior to the initial appointment. Feel free to submit the completed form electronically anytime prior to your appointment, or you may print and bring completed forms with you to your appointment.
